Home Delivered Meals
Access to a Balanced Diet
The Home Delivered Meals Service is a program for seniors who are homebound due to illness, incapacitating disability or who are otherwise isolated. The spouse of the homebound person may also receive this service regardless of age, if it is in the best interest of the home bound person. This service also provides a point of contact for individuals who live alone and may need social contact or help with obtaining other services.
EligibilityHome Delivered Nutrition Services are targeted to persons 60 years of age or older. Priority shall be given to older individuals with greatest economic and social need, with special emphasis on low- income minority individuals, older individuals with limited English proficiency, older persons residing in rural or geographically isolated areas, and older individuals at risk for institutional placement.
The meal must be delivered and received at the home of the eligible individual who is homebound. Homebound is someone unable to leave home to attend regular social activities such as a senior center or congregate nutrition site. The recipient may be able to go to medical appointments, but needs escort assistance to do so safely. A client without access to adequate nutrition and for whom transportation to a congregate site is unfeasible may be considered homebound. |
