At-Home ServicesAt-Home Services are targeted to persons 60 years of age or older who are frail, have disabilities, or who are at risk of institutional placement. Priority shall be given to persons who are in the greatest economic or social need and/or residing in rural or geographically isolated areas, with particular attention to low-income minority individuals and individuals with limited English proficiency. PSR offers four types of services for seniors living at home: Homemaker, Care Coordination, Repair and Renovation, and CHORE.

HomemakerHomemaker services provide assistance to persons with the inability to perform one or more of the following activities of daily living: preparing meals, shopping for personal items, managing money, using the telephone or doing light housework. A mature, trained, supervised aide performs light housekeeping, plans and prepares meals, assists with daily living and teaches individuals how to live and cope with a new disability or to prevent further illness.

Care CoordinationCare Coordination assists individuals who are frail or disabled, or who are at risk of intuitional placement. The Care Coordination program seeks to maintain independent living and for older adults to be self-sufficient. Care Coordination services work with the individual and caregivers to address the needs and goals of the individual to remain in their home. Care Coordination also monitors and coordinates clients' treatment plans, educates them about their conditions, connects them with healthcare providers, and evaluates their progress.
