At Home Services

At-Home Services

At-Home Services are targeted to persons 60 years of age or older who are frail, have disabilities, or who are at risk of institutional placement. Priority shall be given to persons who are in the greatest economic or social need and/or residing in rural or geographically isolated areas, with particular attention to low-income minority individuals and individuals with limited English proficiency. PSR offers four types of services for seniors living at home: Homemaker, Care Coordination, Repair and Renovation, and CHORE.

Homemaker

Homemaker services provide assistance to persons with the inability to perform one or more of the following activities of daily living: preparing meals, shopping for personal items, managing money, using the telephone or doing light housework. A mature, trained, supervised aide performs light housekeeping, plans and prepares meals, assists with daily living and teaches individuals how to live and cope with a new disability or to prevent further illness.​

Care Coordination

​Care Coordination assists individuals who are frail or disabled, or who are at risk of intuitional placement. The Care Coordination program seeks to maintain independent living and for older adults to be self-sufficient. Care Coordination services work with the individual and caregivers to address the needs and goals of the individual to remain in their home. Care Coordination also monitors and coordinates clients' treatment plans, educates them about their conditions, connects them with healthcare providers, and evaluates their progress.

Home Repair and Renovation

​The Home Repair and Renovations program is designated towards seniors 60 years old and older with low-income, risk of institutional placement, residing in rural areas and having certain disabilities. Some examples of services are: door, window, rotten floors, walls, ceiling, and stairs repairs as well as small plumbing, electrical, and heating issues.

CHORE

​CHORE services provide assistance to older adults who experience difficulty with activities of daily living, and is targeted to older adults 60 years of age or older who are frail, have disabilities, or who live in isolated areas, with particular attention to low-income, minority individuals. Services of this program include window washing, floor cleaning, yard maintenance, painting, chopping and stacking wood, removal of ice and snow, heavy cleaning and minor home repair work.
